(Optional) Questions for 801 reporting
Lead Agencies may include these pages if necessary to capture additional information for 801 reporting purposes. All questions are optional for applicants. Including these pages creates additional barriers for families applying. If you are able to add these questions as part of processes that occur after eligibility determination, OCC strongly recommends doing so.
